Review: Medication Error in Prescribing and Dispensing Phases on Outpatient

Background: A medication error is defined as "any preventable event that may cause or lead to inappropriate use patient harm while the in control of healthcare professional, patient, consumer." might occur at any point during medication-use process, such when prescribing medicines, dispensing, and drug taken by patient. Objectives: This review focusing on types errors (MEs) which commonly occurs dispensing phase outpatient Indonesia. Material Methods: Articles related MEs were collected from DOAJ (Directory Open Access Journals) google scholar. The articles reviewed analyze draw conclusions about common type mostly occured outpatients. There 10 (2003-2020) have been reviewed, covering MEs, incidents how reduce number incident. Results: Based articles, incomplete data patient's prescription include address, born date, weight gender; unclear information including, dosage, route adminstration, illegible writing; uncomplete doctor's practise license wasn't listed, there no medicine interactions. use, time using, indication, amount given, side effects, storage instruction, strength doses medicine, writing etiquette, compounding well was what do if forgot take medicines.
